As release volume grew, this created real risk around template compliance, cross-release traceability, and audit-readiness.

  • Audit-Readiness Risk –Documentation lacked a consistent change history and approval trail, making audit preparation slow and reactive rather than continuous.
  • Weak Traceability Across Releases –There was no systematic way to trace requirements through to technical artefacts or detect version and change impact across the SDLC.
  • Inconsistent Template & Standards Compliance –Without a controlled authoring process, documentation quality and structure varied by author, team, and release.
